Acute hypoxemic respiratory failure (low oxygen delivery), chronic respiratory failure and squamous cell carcinoma (skin cancer) of the base of the tongue were listed as underlying causes. Malnutrition and tracheocutaneous fistula (often caused by a tracheostomy to open a hole in the windpipe) were among the contributing factors. Kilmer died in Los Angeles on April 1 at age 65 after an iconic Hollywood career in which his films earned more than $3.
