Pune: PMC 's maternal death review report on Friday cited hypoxic encephalopathy, along with lactic acidosis and and disseminated intravascular coagulation, as the cause of Tanisha alias Ishwari Bhise 's death on March 31 — three days after she was allegedly denied treatment at Deenanath Mangeshkar Hospital on March 28. Pune Municipal Corporation's (PMC) health department on Friday submitted the detailed maternal death review report related to Bhise to the state health department. A senior PMC official, a part of the maternal death review committee, said on the condition of anonymity, "As per the doctors who treated the patient and the documents available with us, we can say that the cause of her death was hypoxic encephalopathy with lactic acidosis (LACS) and disseminated intravascular coagulation (DIC).

The health department's five-member committee first submitted its report, indicting the hospital for not following the "golden hour" treatment protocol. Another senior official in the maternal death review committee clarified that a separate committee was not set up exclusively in this case.
"It is a routine review committee for every maternal death in the city," she said. Bhise had gone to Deenanath Mangeshkar hospital for treatment on March 28. After her relatives were allegedly told to deposit Rs10 lakh, she was shifted to Surya Hospital.
Bhise delivered twins there on March 29, but her condition deteriorated post-delivery. She was then taken to Manipal hospital, where she passed away on March 31. According to experts, Bhise suffered from hypoxic encephalopathy because her brain did not receive enough oxygen and blood.
They said LACS was a metabolic disorder, characterised by a buildup of lactic acid in the body, often because of insufficient oxygen delivery to tissues. DIC is a serious blood clotting disorder. It can be triggered by various pregnancy-related complications, including placental abruption and massive obstetric hemorrhage.
"These events can severely restrict oxygen supply to the brain, resulting in hypoxic encephalopathy. Early detection and management of DIC and the underlying causes, along with appropriate interventions, are crucial to reduce the maternal mortality," said one of the experts. Explaining the role of the maternal death review committee, Dr Rajesh Dighe, assistant health officer, PMC said, "The panel comes into picture after every maternal death is reported.
This committee exclusively works towards finding the exact cause of a death. These deaths are annually reviewed to improve the system and reduce the maternal mortality. This is a fact-finding committee and not a fault-finding committee.
The guidelines specifically state the same." According to PMC's health department, a total of 70 maternal deaths were reported between April 2024 and March 2025. Of these, a total of 26 were residents of PMC limits, 25 were from outside PMC areas but in Pune district and 19 from other districts.
Explanation sought from treating doctor The Maharashtra Medical Council (MMC) on Wednesday issued a notice to Dr Sushrut Ghaisas, the treating doctor of the woman who died after she was allegedly denied treatment at Deenanath Mangeshkar Hospital last month. Dr Vinky Rughwani, administrator, MMC, said, "We received a detailed response from the hospital on Wednesday regarding the incident. Based on media reports and the hospital's statements to us, we have sent a notice seeking explanation from Dr Ghaisas on the entire episode on Wednesday itself.
We have also sent the notice by post." Dr Ghaisas, a former gynaecologist of the hospital, allegedly demanded a Rs10-lakh deposit from the victim's kin before admitting her. Dr Rughwani said, "We have given Dr Ghaisas four days' time to respond to the notice.
We are expecting a reply by Sunday or Monday. We may even call him for a hearing as ours is a quasi-judicial body."
